Research Interests
The SmithLab developed the first patient-derived rectal cancer organoids (PDOs). These RC PDOs recapitulate the clinical and molecular features of patients with RC and uniquely reflect the clinical responses to therapy of the patients from which they were derived. I obtained an R37 to further develop and expand this RC model, and my work as a clinician and clinical trialist completely overlaps with my basic and translational work focused on improving response to neoadjuvant therapies for patients with RC by developing better disease models. I have demonstrated the ability to develop strong collaborative relationships as evidenced by ongoing work with Drs. Romesser, Paty, Garcia-Aguilar and through this unique collaborative initiative we have been able to identify and characterize novel radiosensitizers to take forward into Phase I clinical trials with parallel co-clinical trials in RC PDOs.
Further, we have improved our initial in vivo endoluminal RC model by translating it to the immunocompetent setting to better understand the biology contributing to chemoresistance and radioresistance in RC (PMID 36590695). I am actively working to extend and validate findings from the OPRA trial (PMID:35483010) and our ongoing work in this space (PMIDs: 39808079; 39266364; 39060961) to better identify complete responders to neoadjuvant therapy with the end goal of increasing the number of patients who can benefit from organ preservation. Our RC tumoroid model fills an unmet need by enabling the discovery of robust, cost-effective molecular biomarkers for RC patients. This has led to ongoing R37 funding through 2027 (extension approved). Furthermore, recent work has led to a provisional patent filing as well as a new SBIR grant with Orbits Oncology recently submitted (January 2025). In addition, we are exploring the utility of radiosensitizers to better target tumor downsizing using our models in addition to using normal tissues in a novel manner to expand the therapeutic index and spare normal tissues: https://www.medrxiv.org/content/10.1101/2025.01.15.25320629v1
To translate our pre-clinical results, there is a critical need to develop reproducible, robust, and clinically useful biomarker panels for identifying RC patients most likely to benefit from neoadjuvant therapies. Furthermore, it is imperative to discover new biomarkers for targeted treatments in patients that are resistant to total neoadjuvant therapy (TNT) as some modern trials have shown patients with excellent response whilst others lag behind. We are uniquely positioned to address these needs and identify reliable biomarkers as we can derive patient-specific models pre-treatment and at separate points during treatment to identify the key features of resistant tumors so that we can target these features and then render tumors sensitive. Since tumoroids consist of tumor epithelial cells and we showed that our RC tumoroid model corresponds to TNT of RC patients, the premise of our recently submitted multi-PI RO1 application with long-time collaborator Dr. Xi Chen is that we can maximize the utility of our model to derive effective intrinsic molecular biomarkers for TNT response by strategically combining genomic data from tumoroids and matched primary tumors of RC patients (https://www.medrxiv.org/content/10.1101/2024.01.29.24301906v1).
Lastly, as an active clinical trialist and Chair of the National Phase II now Phase III trial (The Janus Rectal Cancer Trial – opened 9 November 2022 – NCT05610163 (n=776/760 patients (now completed as of 2 July 2025)) that was built on the findings from the OPRA trial, I am well positioned to ask and answer relevant translational and clinical research questions to improve outcomes for RC patients. MSK served as the lead site for the trial and accrued ~66 patients to the trial – notably MD Anderson accrued the last patient on the trial in collaboration with our partners at Ben Taub. Notably, this trial rapidly accrued nationwide and is 100% accrued for the Phase II and the Phase III portion employing disease-free survival as the endpoint. The Janus Rectal Cancer Trial is the largest clinical trial in the world for RC patients, and we are well-positioned to perform and continue parallel co-clinical trial work at both MSK and MDACC.
