About Dr. Ouida L. Westney
The goal of this research is to characterize urinary symptoms in breast cancer patients with a focus on the prevalence and severity in patients who are in the survivorship phase. We plan to utilize validated urinary specific quality of life questionnaires to determine whether a history of anti-estrogen therapy results in an increased prevalence and altered pattern of urinary symptoms in comparison to healthy controls. Secondarily, using additional samples of patients in different phases of treatment, we will develop a natural history of urinary symptoms in patients diagnosed and treated for breast cancer. Identification of the most frequent urinary complaints in these patients will furnish information for revising current breast cancer specific quality of life instruments. Furthermore, we will be able to determine which specific urinary complaints have the most detrimental effect on breast cancer survivors with the goal of focusing treatment in those areas.
Prior work discussing urinary symptoms in breast cancer patients has not been designed in a manner to systematically define the prevalence and severity of pertinent urinary tract symptoms. An organized approach to answering this question requires a multidisciplinary team including a urologist . As a reconstructive urologist with fellowship training in female urology, urinary tract reconstruction and neurourology, I feel that I am particularly suited to investigation focused on urinary dysfunction in oncology patients. My clinical practice at MD Anderson is composed primarily of postprostatectomy incontinence, urinary diversion simultaneous with cystectomy, obstructive voiding dysfunction, female stress urinary incontinence and treatment related (radiation and/or chemotherapy) irritative voiding symptoms. For this particular project and the subsequent investigations, I have constructed a team including experts in urology, gynecology, breast medical oncology, breast surgical oncology, clinical cancer prevention, and symptom research. Each member of this group brings a particular knowledge base critical to a comprehensive approach to defining the prevalence, severity and impact of urinary tract symptoms in breast cancer patients. For the key roles of database construction, database management and questionnaire distribution, I will be utilizing the shared resource, PROSPR (Patient Reported Outcomes, Survey and Population Research). This group has extensive experience with research projects utilizing mail distribution as a primary mechanism for obtaining patient response to questionnaires.
Additionally, I have prior experience directing studies utilizing validated questionnaires as a key component of symptom assessment. I successfully completed a project funded by The University of Texas Medical School that correlated decreased pelvic perfusion to erectile dysfunction and urinary symptoms in atherosclerotic males in comparison to males with negative cardiac catheterization studies using validated questionnaires and dynamic perfusion MRI. In that study, it was necessary to collaborate with our radiological colleagues to utilize an imaging modality in a novel way. Additionally, we worked closely with cardiology to identify and recruit the appropriate patients. This current research proposal has a similar investigative process, despite the difference in gender, based on the utilization of patient reported self-assessment. In summary, I feel that I have the critical clinical background and the prior experience for structuring a project that will generate information designed to serve as a foundation in urinary symptom research in breast cancer patients.
