About Dr. Ye Ethan Li
The unique aspect of my work is the focus on the identification of treatment resistance of anti-cancer therapy and development of the novel strategy to overcome the therapeutic resistance and enhance treatment efficiency, as well as the translation the novel treatment strategies to the clinic, which centers on the expansion of immune cells for the purpose of immunotherapy. I have explored the understanding of the molecular etiology and immune response in the process of malignant transformation, applying cellular-based immune therapies to increase curative efficacy in treating cancer. Over my 10 years lab research experience in leading different scientific projects, I have succeed in accumulatively publishing research articles as the first author relevant to cancer biology (Oncotarget, 2017), cancer bioinformatics (Nutrient. 2021), and anti-cancer cellular therapy (Cell Stem cell, 2018; Nature Medicine, 2022; Nature Medicine 2024). With my current position, I gain more access to the clinical data and clinical data analyze that allows me to integrate the question found fresh from the bed to the solution in bench. I aim to investigate the understanding of biomarkers during malignant progression as well as their treatment resistance mechanism with advanced cellular therapeutic approaches in order to develop novel strategies to improve outcomes for patients with cancer.

Research Interests
Specifically, in the most recent 5 years, my research in advancing anti-tumor cellular therapy by using NK cells has achieved great impact on both healthcare industry and the field of anti-tumor cellular therapy and immunotherapy.
(i) Development of induced pluripotent stem cells (iPSCs)-derived natural killer (NK) cells with the expression of chimeric antigen receptors (CARs) that employ NK–specific activating signaling. The engineering of CARs may significantly enhance the anti-tumor activity of immune effector cells. NK cells have shown anti-tumor activity with no severe clinical toxicity. However, there is a lack of CAR signaling designated for optimizing NK activation. My research focuses on the re-construction of CAR signaling by employing NK-specific activation domains. Based on the cellular platform of iPSC-derived NK cells, I identified a CAR containing the transmembrane domain of NKG2D, the 2B4 co-stimulatory domain, and the CD3ζ signaling domain, which, together, mediates a strong antigen-specific NK cell response. In an ovarian cancer xenograft model, my work showed that NK-CAR-iPSC-NK cells significantly inhibited tumor growth, and prolonged survival compared with peripheral blood (PB)-NK cells, iPSC-NK cells, or T-CAR-iPSC-NK cells with less toxicity. These NK-CAR-iPSC-NK cells have been used in clinical trial as a type of "off-the-shelf" immune cell therapy in treating patients with B cell-derived (NCT04245722), which achieved in 74% response rate in treatment patients with B cell malignancies in the Phase I report. By the time of drafting this curriculum, the article describing my leading research entitled “Human iPSC-Derived Natural Killer Cells Engineered with Chimeric Antigen Receptors Enhance Anti-tumor Activity” has been cited for more than 500 times from peers of more than 50 institutes in 40 countries and territories,
(ii) Identification of trogocytosis (TROG) as a treatment resistance of CAR-NK cell-mediated antitumor therapy and the development of a novel dual-CAR system overcome the TROG-antigen-induced on-target off-tumor effects on CAR-NK cells, while sparing their on-target anti-tumor activity. In my study, I revealed a unique mechanism for tumor escape and treatment resistance following CAR-NK cell therapy; specifically that activation of activating CAR (aCAR) in NK cells drives the transfer of CAR-cognate antigen on NK cells from tumor cells that leads to NK cells fratricide and dysfunction. This phenomenon is associated with a concurrent loss of target antigen on the cancer cells, also rendering in tumor escape. Importantly, my designing of the novel dual-CAR strategy on NK cells that combines an extracellular domain targeting an NK-specific antigen with the signaling endodomain from an inhibitory KIR (iCAR) successfully that provides a “don't kill me” signaling, and achieves antigen-specific CAR-NK activation and cytotoxciity against tumor, but ignore the antigen expressing CAR-NK effector cells. This dynamic modulation of AI-CAR-signaling may find useful applications to improve the in vivo persistence and therapeutic efficacy of a range of adoptive NK cell therapies. As the first author, my manuscript entitled “KIR-based inhibitory CARs overcome CAR-NK cell trogocytosis-mediated fratricide and tumor escape” was published in the highest-esteemed top-ranking journal of Nature Medicine, 2022. Tested in a phase I-II clinical trial using allogeneic umbilical cord-derived NK cells and genetically modified with CAR targeting at CD19-positve B cell-derived malignancies (NCT03056339), the higher TROG-antigen accumulation on CAR-NK cells significantly associated with non-responders of treatment and also contribute to the early tumor relapse, the strudy of which were published in Nature Medicine 2024. Both of my first author publication were selectively presenting in the “Wall of Science FY23Q1, FY24Q2” by MDACC.
